Required Forms

FORMS: DO NOT fill out these forms until after you have a confirmed appointment scheduled. For general inquires about appointment avialability and to schedule an appointment please fill out the 'Contact Me' form at the bottom of the website main page. At least 24 hours before your scheduled appointment complete the Patient Intake Form below (form will not load if you are using Chrome).  If you click on the Forms Online link it will open a new window on FormDr.  If you prefer paper documents, you can fill out the PDF version of the paperwork and email it back.

TELEHEALTH INSTRUCTIONS: Telehealth sessions are conducted via a secure web-based platform.  All data is encrypted, your sessions are anonymous, none of your information is stored and it is HIPAA compliant. Click here for instructions and important information for a quality connection. When you are ready for your session click on the link below or type the URL (https://doxy.me/dralisonparsons) into your browser.
